A car runs the best when four wheels are perfectly aligned and balanced, isn't it? Imagine your mouth as a four wheeler. The mouth is also divided into 4 quadrants, upper and lower as well as right and left side, each having 7 sets of teeth. Unlike natural teeth, dental implants are not susceptible to dental decay; however, the health of the gums is vital to maintain lasting implant success.

 There are 3 aspects to the longevity of your dental implants.

There are two types of implant failure.

Immediate implant failure– Failure of the dental implant 4-90 days after implant placement. This could be because of a number of reasons but the most common and the most preventable are infection and bone loss. It’s also possible that the implant may fail to osseo-integrate with your jawbone. At our Clinic we have a 98% initial success with dental implants.

Delayed implant failure-This is also known as Peri implantitis- a kind of infection that forms around the implant and the gums. This is usually due to poor dental hygiene maintenance after an implant placement. At our office, we have made a strict rule to attain regular checkups at-least once in a year to avoid any complications. Implant therapy may not be possible if the jawbone is not wide enough to accommodate implants and bone grafting is not possible.

Untreated periodontitis or uncontrolled diabetes makes it difficult to place implant because of the risk of failure due to infection or poor healing. Such conditions needs medical attention and once treated favorably, implants can be performed.

Implants are also not suitable for people who take medication for osteoporosis – bisphosphonates or Denosumab – as they are at higher risk of developing necrosis of the jaw bone. Patients taking antidepressent drugs or with poor Vit D3 level possess significant risk of failure. Such patients need a consultation before implant placement.

Other medical conditions that rule out the use of implants include:

  • Blood-clotting disorders.
  • Deficiencies in the immune system.
  • Severe systemic illnesses and drug abuse.

Smoking is another important consideration, as smokers have a greater risk of developing peri-implant diseases and are more likely to experience loosening of their implants. Quitting smoking will significantly increase your chances of successful implant therapy and achieving long-lasting results.

With the sedation techniques available at the time of treatment and helpful instructions for home care, many people report that they feel better than anticipated. Often, they return to work and daily activities the same day or the next day. While there may be some discomfort and bruising, this typically lasts only a short time.

Treatment time varies from patient to patient depending on quality and quantity of available bone to place the implant. Dental implants can be placed within one day. Typically, you will have some type of temporary teeth until your implants integrate with the bone. Usually we prefer to avoid loading them immediately for the long term success and restore the implants within 3-4 months’ time. However, It is important to consult your implantologist and plan your trip accordingly.

Patients travelling from overseas should be in India for a minimum of 12-15 days for the surgical treatment to allow us to check for any possible complications. Should complications arise after you have returned home, you would either have to fly back to India so that we could take care of your problems or you would have to consult a local periodontist at your own cost.

The most common causes of implant failure are poor oral hygiene, history of periodontitis, and smoking. If tooth cleaning is poor, bacteria will build up on the implant surface and cause inflammation of the surrounding mucosa – which is known as peri-implant mucositis.

If left untreated, mucositis can progress to a more dangerous condition called peri-implantitis, in which there is greater damage to the bone and tissue around the implant. Eventually, the implant becomes loose, which needs to be replaced – and this could require additional bone grafting and regenerative procedures to develop new bone.

Peri-implantitis is usually much more difficult to treat than periodontitis.

Warning signs of peri-implant diseases include:

  • Increased tendency for the gums to bleed during brushing and interdental cleaning.
  • Swelling of the gums.
  • Bad breath or a bad taste in your mouth.
  • The loosening of the implant, bridge, or dentures.

Regular check-up visits with your dentist will help ensure that any problems are picked up early and thus help prevent their onset. If you notice any signs of peri-implant disease mentioned above, it is vital you make an appointment with your dentist or periodontist as quickly as possible.
